Payment Information
Since many companies require prior authorization before you can be seen, please check with your insurance carrier before your first appointment. You will need to bring your insurance card and co-payment with your to your appointments and, unless otherwise arranged, payment is expected at the time of each visit. We accept payment in the form of cash or credit cards.
